Tinnitus and Earwax Buildup: A Fixable Cause
Before working through more involved causes of tinnitus, it's worth ruling out one of the simplest: impacted earwax (cerumen). It's an unglamorous explanation, but it's genuinely common, easily checked, and — unlike most tinnitus causes — often fully resolved in a single visit.
Why earwax can cause tinnitus
Earwax buildup can press against the eardrum or block sound from reaching it normally, producing muffled hearing and, in some people, tinnitus as a secondary effect. The mechanism is thought to be similar to other conductive hearing changes: when sound transmission through the ear canal is disrupted, the auditory system can respond with the same kind of compensatory activity implicated in other forms of tinnitus (see the article on tinnitus and hearing loss for the underlying mechanism). Signs it might be earwax rather than something else
- A feeling of fullness or blockage in the ear, sometimes coming on gradually
- Muffled hearing, particularly if it started suddenly or after swimming, showering, or using cotton swabs
- Tinnitus that appeared alongside the fullness or muffled hearing, rather than on its own
- Symptoms in one ear rather than both, if the buildup is asymmetric
None of these signs is definitive on its own — an audiologist or doctor can confirm with a simple otoscope exam, which is quick and non-invasive.
A nuance worth being upfront about
A large US national health survey (NHANES data, over 14,000 participants) found cerumen impaction is genuinely common — affecting close to 1 in 5 adults, and roughly a third of adults over 70 — and confirmed a measurable link between complete impaction and reduced hearing. It did not, however, find a statistically significant association between cerumen impaction and tinnitus specifically, once other factors were accounted for. That's a more rigorous, population-level finding than the case-by-case clinical reasoning above, and it's worth stating plainly rather than glossing over: earwax causing tinnitus in a given individual is biologically plausible and does happen, but it isn't a strong, consistent pattern across the population the way, say, noise exposure is. This doesn't change the practical advice below — checking is quick, free of downside, and genuinely resolves the tinnitus for some people — but it does mean earwax shouldn't be assumed as the likely explanation without checking.
Why it happens
Earwax is normally self-clearing through jaw movement and skin migration in the ear canal, but a few things commonly interfere with that process: frequent cotton swab use (which often pushes wax deeper rather than removing it), hearing aid or earbud use (which can block the canal's natural clearing mechanism), naturally narrow or curved ear canals, and simply aging, since earwax tends to become drier and more prone to impaction later in life.
What to actually do about it
This is one tinnitus cause with genuinely simple, low-risk fixes:
- See a doctor or audiologist for professional removal if you suspect impaction — they can safely remove it via irrigation, suction, or manual extraction, and confirm whether wax was actually the cause once it's cleared
- Avoid cotton swabs inside the ear canal — they're a leading cause of impaction, not a solution to it, despite how commonly they're used for this purpose
- Over-the-counter softening drops can help with mild buildup, though professional removal is more reliable for significant impaction and avoids the risk of pushing wax further in
- Don't attempt ear candling or other unproven home remedies — these aren't supported by evidence and carry a real risk of burns or further blockage
The genuinely good news here
Unlike most causes covered on this site, earwax-related tinnitus often resolves completely once the blockage is cleared — making it worth ruling out early, simply because it's the fastest possible resolution if it turns out to be the answer. It sits on the list of tinnitus that can actually be cured as the quickest entry by some distance — the others on that list need surgery, a medication review, or months of treating a jaw disorder, and this one is an appointment. If tinnitus persists after wax removal, that's useful diagnostic information too: it means the earwax likely wasn't the primary cause, and it's reasonable to move on to a fuller audiological evaluation from there.
