Tinnitus in Musicians: Occupational Risk and Prevention

Musicians occupy an unusual position in tinnitus risk: the noise exposure is chronic, close-range, and often central to the job in a way that isn't true of most other high-noise occupations. Unlike a construction site, you can't simply put in earplugs and step away from the source — the source is what you're being paid to produce or closely surrounded by.

Why the exposure is different

Sound levels in rehearsal and performance settings routinely exceed the thresholds associated with hearing damage: orchestral brass and percussion sections regularly measure well above 100 decibels at close range, amplified rock and pop performances often exceed 110 decibels, and even acoustic ensemble rehearsals in small rooms can reach damaging levels through reflected sound. Unlike a single loud concert as an audience member, musicians accumulate this exposure across rehearsals, sound checks, and performances, often multiple times a week for years or decades.

Position matters too — string players sitting in front of brass sections, drummers, and anyone near a monitor speaker face meaningfully higher exposure than musicians positioned elsewhere in the same ensemble, which is part of why hearing damage isn't evenly distributed even within one group.

What the risk actually looks like

Tinnitus and hearing loss among musicians follow the same underlying mechanism as any noise-induced case (see the dedicated article on noise-induced tinnitus) — cumulative hair cell damage from repeated loud exposure. What's specific to musicians is the difficulty of avoiding it while still doing the job: hearing protection has historically been seen as incompatible with performing, since standard earplugs muffle the detail and balance a musician needs to hear their own playing accurately.

A 2026 meta-analysis pooling 67 studies and over 28,000 musicians across 21 countries found 42.6% reported tinnitus, compared to 13.2% in matched control populations — more than three times the rate. The same analysis found hearing loss in 25.7% of musicians versus 11.6% of controls, and hyperacusis in 37.3% versus 15.3%. One detail worth knowing if you assumed genre mattered: the researchers found no significant difference in tinnitus, hearing loss, or hyperacusis rates between classical and pop/rock musicians — the risk is consistent across genres, driven more by cumulative exposure, instrument type, and seating position than by the style of music itself.

Where the reviews disagree, and why we are not picking a side

That genre finding is worth flagging as contested rather than settled. An earlier systematic review — 41 studies covering 4,618 professional musicians — reached the opposite conclusion, reporting hearing loss in 63.5% of pop/rock musicians against 32.8% of classical musicians, and attributing the gap to higher-intensity exposure and less consistent ear protection. Its overall hearing loss figure, 38.6%, is close to the newer pooled estimate; the disagreement is specifically about whether genre explains part of the risk.

Two reviews of overlapping literature reaching opposite conclusions on the same question usually means the answer depends on which studies get included and how "pop/rock musician" is defined across them — a session player, a touring act and an orchestral percussionist are not the same exposure. The practical implication does not change either way: cumulative exposure, proximity to loud sections, and whether protection is actually worn matter more than the label on the genre.

A separate Norwegian survey of 111 active rock musicians gives a useful third data point in the same range: hearing loss in 37.8% and chronic tinnitus in 20%. It also found what most of this literature finds — custom-fitted earplugs have a preventive effect, and only a minority of musicians use them consistently.

What actually helps

  • Musician's earplugs, specifically — unlike foam earplugs, these use flat-attenuation filters that reduce volume relatively evenly across frequencies, preserving tonal balance rather than just muffling everything. They're a meaningfully different product from standard hearing protection and worth the cost difference for regular players.
  • In-ear monitors set at reasonable levels, which can reduce the need to compete with loud stage volume by delivering a clear mix directly, rather than musicians turning amps and monitors up further to hear themselves over each other.
  • Positioning and acoustic screens, particularly for musicians seated near brass, percussion, or amplified sections — physical barriers and spacing make a measurable difference where feasible.
  • Regular hearing checks, ideally with a baseline audiogram early in a music career and periodic rechecks, so early changes are caught before they progress — particularly relevant given how gradual and easy to normalize this kind of hearing change can be.
  • Volume-limiting practice habits at home or in individual practice, where exposure is fully within a musician's own control, unlike ensemble or performance settings.

The tradeoff musicians actually face

Hearing protection for musicians is a genuine balancing act between long-term hearing preservation and short-term performance needs, not a simple "always wear protection" recommendation — which is exactly why musician's earplugs and in-ear monitoring exist as a middle path, rather than treating standard hearing protection and no protection as the only two options. For anyone making music a significant part of their life or livelihood, treating hearing protection as professional equipment — on the same level as an instrument or amp — rather than an afterthought is the practical shift that tends to matter most.
